Card Protection Premier Terms and Conditions
 
• Your offer to purchase the Card Protection Premier is subject to the acceptance by Card Protection Plan Limited ("CPP").
• Details of Card Protection Premier are set out under the Membership Terms and Conditions which will be provided by CPP upon approval.
• The Membership and annual renewal fees will be paid via debiting your Citibank Credit Card Account with such amount and at such times as may be instructed by CPP.
• Card Protection Premier is provided by CPP. The role of Citibank is limited to introducing you to Card Protection Premier only. Citibank is not responsible for any matters relating to Card Protection Premier, including cancellation of Membership. You are to settle any dispute and matters regarding Card Protection Premier directly with CPP.
• The Citibank Card Agreement Terms & Conditions shall continue to apply.
• For any replacement of the Citibank Credit Card as a result of loss, fraud, or upgrade, Citibank will impose a charge to the replacement card.
• By signing up for Card Protection Premier, you are deemed to have understood and agreed to be bound by the terms set out in this form and the Membership Terms and Conditions of Card Protection Premier.
• In addition, by signing up for Card Protection Premier, you also expressly consent to Citibank's disclosure of your name, IC no., credit card no., address, telephone number and other personal information that Citibank deems necessary to CPP, CPP's branches, its affiliated companies and agents, for the purposes of processing new application or renewal of Card Protection Premier and all other purposes relating thereto.
